it should be the same  settings as reel 1 and 2.

Just did what you have suggested ran a few quid through got 3 numbers in a row still pops up with the same issue,as you can see inverted opto is unchecked on reel 3 ,its a strange one.

Reinstalled mfme 20.1 ran a few quid through it still pops up with the same issue ,even looked at the properties on reel 3 in edit as suggested in another old topic but nothing unusual i can see, i am stumped

Apologies in advance if this is the wrong place as im trying to find my way around , been playing Pooks Jiggin The Riggin every time it hits 3 numbers in a row it chucks up OPT FAULT ON REEL 3, love to sort it out any help would be great
